Please find attached my CV for your review. To provide you with a condensed history,
let me offer the following. I have worked in medical/ surgical offices most of my career.
I first started with George Waters, M.D. He was a very talented surgeon. His practice
was small, but provided excellent care. He has since retired and decided to close his

an O.D. who left when his wife transferred. Very soon they wanted me full time, so I
sold my own practice and joined the office full time. After a number of years, William
Whitson, M.D. from PriceWhitson Vision Group Corneal Consultants, who had been
seeing patients at the Lafayette Eye Center I did all of his postop care asked me to
join them full time. As prestigious as the practice was, I could not pass up the
opportunity. Sadly, only months after joining them, the group split and all staff had to go
with Dr. Price. He had opened an office in Lafayette, IN that was originally to be just a
true satellite location, but was converted to a low cost LASIK facility where we trained
the fellows in the practice on the LASIK procedure. The first years the practice was
open we did nearly eyes with an average postop vision of better than /. After
the / disaster, business for a one dimensional luxury item fell off and the practice
closed. In all, he closed four of five locations and eliminated all but two parttime O.D.’s
at one point there were doctors combined and Fellow.
I joined an optometry practice that the doctor claimed could support three doctors. After
getting there, the practice was significantly different than promised. My income dropped
by %. The next spring Lafayette Eye Center called me and asked me to return. I did,
at one day per week. By the third week I was booked out many weeks so they returned
me to full time. In , at the age of , Dr. Sharvelle wanted to begin a retirement
process, so he sold the office to a large group practice. At the time the optical
generated over $M annually. The new owners very quickly closed the optical and the
surgeon to replace Dr. Sharvelle was hired without even seeing a surgery. He was
dismissed several months later. As the practice continued to deteriorate due to lack of
support from the owners, they announced it would close in July and everyone was

dismissed. Not long afterward, the CEO was charged with embezzling and their entire
practice went into bankruptcy. Soon after that, one of the surgeons sued the group for
not paying him and the company was broken up in bankruptcy court and sold off in
At this point, my wife Donna and I opened our own practice. The recession had started
and there were no jobs available. While we didn’t like the idea of owning our own
practice again, there were few options. I love seeing patients but do not enjoy the
business parts of a practice such as billing, payroll, accounting, etc. During this time,
my wife was a paraprofessional at a local school corporation but because of major
budget issues, most of the support staff was let go. Being a small practice, we could
not buy health insurance at a reasonable cost. We stayed on COBRA for a time but
that ran out starting January . We decided it was time for change. We got an offer
to work for Buckeye State Optometry Associates, who contract to one of the major chain
opticals and decided to sell our office, or close it. Fortunately, a group that has multiple
locations wanted the practice and made a reasonable offer for it.
Currently I am in a suburb of Cleveland, OH. We have found the community to be very
nice people. Unfortunately, the practice is not what was portrayed. While the pay and
benefits are excellent, other aspects of the practice just do not match my style.
I am resuming my search for a practice opportunity that will offer me the chance to
provide good medical / family eye care in a professional setting. While I am currently
licensed in Indiana and Ohio, I have checked and it will not be hard to become licensed
in Kentucky, Tennessee, or Michigan we wish to stay in the lower / of Michigan. I

contact anyone connected with my current situation as they are not aware of my plan to
leave this practice environment. While I do not like the situation, it is necessary to keep
it intact to pay my bills until a better opportunity presents itself.
