RESPITE SPECIALIST AND SCL CASEWORKER 

Provided and ensured respite services are provided to individuals served by Lutheran Services in Iowa as requested by families, and other key stakeholders to allow the primary caregivers relief from the responsibility of caring for a child and/or adult with a disability.  Other duties include coordination of services for families, certification of providers including home studies, recertification of current providers. 

Service delivery consistent with the goals, allocated hours and financial limitations of the individuals served.  Written detailed service plans and reports which effectively summarize the services provided by Lutheran Services in Iowa within given time frames.  Facilitate meetings of potential respite providers and the individuals served in a manner which encourages positive relationships. 
 

Instruct, supervise, and assist consumers in the development of daily living skills including but not limited to leisure, time and money management, good health, safety, work and social skills and habit development in accordance to Lutheran Social Services Policy.  

Respite Specialist Case management is a collaborative approach to providing and coordinating health care services. A nurse case manager will works with you, your family, physicians and other health care providers to ensure you or your family member receives all the necessary services to promote your health and oversee any illnesses or health conditions while individual with a disability continues to live at home.

Respite Specialist Case managers identify and engage clients as a result of outreach or referral activities, he or she conducts a facetoface comprehensive assessment with each client of that client’s strengths and limitations and of the social, financial, and institutional resources available to the client. The social work case manager focuses particularly on how these resources relate to the principal concerns identified during the assessment. On the basis of this assessment, the social worker develops an individualized service plan with the client that identifies priorities, desired outcomes, and the strategies and resources to be used in attaining the outcomes. The responsibilities of the social worker, the client, and others should be clarified throughout development of the plan. The direct contact between social worker and client is essential to effectively accomplish the assessment and service plan development.

Work with the client to develop appropriate case plans and goals. Provide ongoing assessment and support.    Assist clients to utilize community resources to meet individual needs.    

This position interviews, coordinates, to meet the social and emotional needs of their clients.  Work with the client to utilize community resources to meet individualized needs of the client. Typically reports to a senior case manager or director. 

Receives and responds to verbal inquiries and/or complaints by telephone while providing account maintenance for a variety of Lutheran Services in Iowa client’s and potential clients.  

Determines the best method to assist customer and resolve problems to ensure customer satisfaction, according to department guidelines and/or client specifications. 

Organizes tasks and resources to complete work and meet deadlines according to established departmental procedures. 

Reads, interprets and maintains current information on client specific data by utilizing various resources including publisher screens, voice mail, memos and web sites.
